A day in the life of a Maintenance Technician.

SASH Wellness Nurse

The Wellness Nurse is responsible for provision and/or oversight of wellness care and coaching for SASH participants in accordance with Vermont's Nurse Practice Act. S/he is responsible for overseeing the well-being of participants and in coordinating health services with other members of the SASH Team and other community providers. S/he supports a philosophy of aging in place consistent with the mission of SASH. The Wellness Nurse will also work with the SASH Team on developing individual participant Healthy Living Plans as well as Community Healthy Living Plans.

Qualifications

The Wellness Nurse must be currently licensed as a Registered Nurse in the State of Vermont. Must possess at least two years of experience in a clinical setting, committed to an aging in place philosophy. He/she must have demonstrated leadership skills and the ability to exercise sound judgment. He/she must have knowledge of standard record keeping procedures (i.e. progress notes, HIPAA guidelines). Must be able to work independently and part of a team and possess strong organizational and time management skills and excellent verbal and written communication skills.

SASH Implementation and Quality Specialist

The SASH Implementation and Quality Specialist works closely with the SASH Assistant Director, Health Systems Educator, and Training Coordinator to ensure the SASH model is implemented consistently and accurately throughout the state. This includes reviewing data, conducting panel reviews with staff in Vermont and Rhode Island, supporting quality improvement projects as needed, and working with SASH staff as well as their supervisors. Additionally, this position provides leadership and support to non-Cathedral Square SASH panels within the Cathedral Square Designated Regional Housing Organization region.

Qualifications

A Bachelor’s degree in Administration, Human Services, Business, or other related field or relevant experience is preferred. At least three years’ experience in quality assurance, technical assistance and training preferred. Direct experience in the SASH model preferred. Must possess excellent interpersonal communication skills and the ability to work with a variety of people in a variety of circumstances. Must be comfortable with technology and enjoy taking on a challenge, be a team player who enjoys multi-tasking, and have excellent written and verbal communication skills. Knowledge of long-term care delivery, affordable housing, and health care reform a plus. Knowledge of Web-based training platforms Excel, Word and PowerPoint necessary. Must possess a valid driver’s license. Some travel is required for this position.

The Housing Manager is responsible for overseeing the building and rental operations at their assigned properties, including: building a trusting relationship with residents, creating a sense of community among residents, and maintaining compliance of all regulatory requirements.

Qualifications

Resident Assistant

Resident Assistants provide compassionate care and support to Cathedral Square residents who are physically and/or cognitively impaired. They work as part of a close-knit, casual and professional team at Cathedral Square Assisted Living in downtown Burlington and/or at Memory Care at Allen Brook, our specialized memory-care community in Williston. They help residents maintain a high quality of life by providing an open ear and helping them with personal care and daily activities, including dressing, eating, hygiene and medications. Experience working with elders is preferred but not required, as we provide paid training!

NOTE: Special precautions are in place at this time to ensure the safety and health of our staff and residents, including additional sanitation, restricted access and social distancing.

Qualifications

  • Compassion, positive attitude, strong work ethic, reliability, and ability to “go with the flow” as needed
  • Excellent interpersonal skills and ability to relate to a variety of people
  • Able to think, act and intervene appropriately in both routine and emergency situations
  • Able to lift, carry, stand, bend, squat, crawl, reach and kneel using good body mechanics for sustained periods of time (think of the steps you’ll get in!)
  • Able to communicate effectively in writing for the purpose of documentation and keeping records

Lead LPN at Memory Care

Provides care and services to residents who are functionally, physically or socially impaired within the scope of license at our Memory Care Residence, under the direction of the RN. Provides charge nurse duties to include medication management and administration, assessment, treatment, and supervision of direct care staff. The LPN supervisor will also be required to provide clinical and scheduling on call two/three days a week, as designated by Manager and RN. S/he supports a philosophy of aging in place consistent with the mission of assisted living and in compliance with the State of Vermont Level III Assisted Living/Residential Care Home regulations.

Qualifications

Must possess a Vermont LPN license. Experience in Long Term care setting preferred, but willing to train the right person. Able to think, act and intervene appropriately in both routine and emergency situations. Must possess a high degree of interpersonal communication skills to relate with a wide variety of individuals. He/she must have demonstrated leadership skills and the ability to exercise sound judgment. Aging in Place Home Modification Technician

The Aging in Place Home Modification Technician (HMT) will be responsible for making the vast majority of home modifications identified by the participant and Occupational Therapist as part of the Older Adults Home Modifications Grant Program (OAHMG) funded by the U.S. Department of Housing and Urban Development (HUD). The HMT will review OT assessment and home modification recommendations for feasibility, and visit participants’ homes to discuss work details and logistics. The HMT will attend regular meetings to coordinate schedule of work, procure supplies for job, and complete the modifications work. As appropriate, HMT will coordinate and contract outside contractors to complete needed home modifications.

Qualifications

High school diploma or equivalent and at least 3 years of experience in property home maintenance. Carpentry skills are required along with general knowledge of plumbing and electrical. Electrical or plumbing license preferred. Knowledge of general health and safety codes related to home modification work a must. Must be able to work with minimal supervision. The ability to respond appropriately to challenging situations and display good decision-making abilities is critical to the success of this position. Superior time management and multi-tasking skills are essential. Must have a valid Vermont driver’s license and reliable transportation. Ability to perform physical activities associated with home modification and maintenance work is required.

Memory Care at Allen Brook Scheduler

The scheduler supports all scheduling functions to ensure adequate staffing to cover Memory Care at Allen Brooks 24/7 community operations. The scheduler also supports Resident Assistants and provides on call coverage when needed. When needed or as scheduled, the scheduler supports residents who are functionally, physically or socially impaired and who need 24-hour oversight. They are responsible for assisting residents with activities of daily living (eating, transfers, toileting, bed mobility, dressing, bathing and ambulation).

Qualifications

A high school diploma or equivalent is preferred. LNA certification is preferred, but not required. Prior work experience as a nursing assistant or aide to people who are elderly or disabled is preferred. Must be able to think, act and intervene appropriately in both routine and emergency situations. Must possess good interpersonal communication skills and be able to relate to a wide variety of people. Must be able to communicate effectively in writing and verbally. Must be flexible and adaptable to changes in the work environment, including scheduling changes and work assignments.
